Geriatric

The Hospital of Central Connecticut provides comprehensive inpatient and outpatient care and services to meet older adults' unique needs.

Nurses and social workers specializing in discharge planning may help arrange care after discharge from the hospital. This may include a rehabilitation or extended-care facility, home care or other services.

The Connecticut Center for Healthy Aging is a resource and assessment center designed to enhance access to services and information related to attaining optimal quality of life for seniors and their caregivers. Staffed by two registered nurses specializing in services for older adults, the Center helps more than 800 seniors and their families annually.
