Regional Director, Care Coordination

Position Duties:
  Under the general supervision of the Central Region Vice President for Medical Affairs, is responsible for the daily operations of inpatient care coordination and social work services. Provides leadership, direction and guidance to staff in the provision of care coordination and social work services. Assists the Central Region Vice President for Medical Affairs in achieving the goals and strategic objectives of the department, and the hospital, region and system.
Educational Requirements:
  •Bachelor Degree required
•Advanced Degree preferred (MBA/MPH/MSN/MS)
•Certification in Case Management preferred (ACM, CCM or CPUM)
Minimum Requirements:
  •Must have a CT RN License
•Recent acute care experience
•Minimum of five years recent experience in hospital case management and utilization review.
•Minimum of three years managerial experience in hospital setting.
•Strong experience in new program development.
•Ability to motivate and facilitate activities of disparate and multi disciplinary groups.
•Strong leadership skills.
•Well developed management & communication skills.
•Motivated, self starter, enthusiastic personality.
•Excellent prioritization and problem solving skills.
•Computer, analytical and financial analysis skills.
•Diplomacy, rapport with physicians.
•Ability to direct daily work in team configuration.
